TeleForm® is a paper‐based document capture tool, which hand print, machine print, optical mark, barcodes and signatures and extracts information rom any document type including semi-structured and unstructured documents.
Teleform provides a robust set of data quality tools and rules that can be incorporated into the design of a form (ranges, default values, dictionary and database lookups). Form processing workflow includes conversion of the form from paper to TIFF (fax or scan), evaluation and verification of the electronic form and data, and data export to a research specific database. This process permits correction of OCR errors, missing data and invalid responses. Teleform security provides assignment of ownership to scanned forms permitting protection of any PHI collected on a form.
